Welcome to our practice!
After your phone call to set up your initial appointment, Linda , our Treatment Coordinator will mail you a welcome letter, a patient information form, and a map to our office.
Please fill out the patient information form at your leisure and bring it with you or you can print it out from the web site and bring it in with you.
Upon your arrival , Linda will escort you to the exam room and take a set of complementary digital photos of you, and introduce Dr. Berry.
This initial appointment will include a preliminary examination to determine if there is a need for orthodontic treatment. We understand that each patient comes to our office with different needs, concerns and expectations about orthodontics. It is for this reason we set aside time to allow us to get to know you and for you to get to know us. Dr. Berry will discuss what treatment is indicated and what your options are. She will also review your photos with you to give you a better sense of any problems that might be present. There is no charge for this visit.
Please allow 45 minutes for your first visit. If treatment is indicated, additional diagnostic records (x-rays, and models) may be done. This information will allow Dr. Berry to determine a specific treatment plan for you. Your next appointment will be a consultation with Dr. Berry and Linda to review your records and discuss in detail, your treatment needs. This also allows an excellent time for us to answer any additional questions or concerns you may have.
Linda will be happy to review the financial aspects of orthodontic care. We offer many affordable payment options and we will be happy to help determine your insurance coverage if applicable. In order to assist in receiving your maximum orthodontic insurance benefit , our office will process and submit your claim.
